The combination of alcohol and ED medication is one of the most common real-world scenarios that prescribing information barely addresses. The Dose Thresholds
A "standard drink" = 12 oz beer (5%), 5 oz wine (12%), or 1.5 oz spirits (40%). Many cocktails contain 2–3 standard drinks in a single glass.
Strategy: Take the Pill First, Drink After
The most practical approach for a night that includes both alcohol and potential sexual activity:
- Take the PDE5 inhibitor on an empty stomach, 60–90 minutes before you expect to drink
- By the time alcohol arrives, the medication is already absorbed and at or near peak levels
- Limit to 2 drinks maximum for best results
- Hydrate between drinks to counteract dehydration (which worsens hypotension)
For men who frequently combine alcohol and sexual activity, daily tadalafil eliminates the timing issue entirely — the drug is always present at steady-state levels, so alcohol's effect on absorption is irrelevant. The arousal-suppression and hypotension effects still apply, though.
Alcohol and ED: The Bigger Picture
Beyond the acute interaction, chronic heavy drinking is itself a major cause of ED. Alcohol-related liver damage impairs hormone metabolism, chronic alcohol exposure damages peripheral nerves, and heavy drinking is associated with testosterone suppression.
