Erectile dysfunction doesn't affect all Americans equally. The states with the highest rates of obesity, diabetes, and smoking — the three biggest modifiable risk factors for ED — also have the highest estimated ED prevalence.

The Geography of ED

While no national registry tracks ED by state, researchers can model prevalence using the strong, well-documented correlations between ED and its primary risk factors. The pattern is striking and consistent.

Highest estimated ED prevalence: Mississippi, West Virginia, Alabama, Arkansas, Louisiana, Kentucky, and Oklahoma. These states consistently rank highest nationally for adult obesity (35-40%+), diagnosed diabetes (12-15%+), and smoking rates. Each of these factors independently increases ED risk by 50-100%.

Lowest estimated ED prevalence: Colorado, Hawaii, Utah, Massachusetts, California, and Connecticut. These states have the lowest obesity rates (22-28%), lowest diabetes prevalence, and the most active populations.

Why It Correlates

Obesity accounts for the largest share of the geographic variation. Men with a BMI over 30 have approximately 1.5 to 3 times the risk of ED compared to men at a healthy weight. Obesity drives ED through vascular damage, hormonal disruption (lower testosterone, higher estrogen), and chronic inflammation.

Diabetes is the single strongest medical risk factor for ED. Men with diabetes are 2 to 3 times more likely to develop ED, and they tend to develop it 10 to 15 years earlier. Diabetes damages both blood vessels and nerves critical for erection.

Smoking doubles the risk of ED. Nicotine is a direct vasoconstrictor, and the chemicals in cigarette smoke damage the endothelial lining of blood vessels throughout the body.

What You Can Control

Your zip code doesn't determine your destiny. The same modifiable risk factors that explain the state-by-state variation are the ones you can change: maintain a healthy weight, manage blood sugar, quit smoking, exercise regularly, and manage stress.
